1. A. A consent agreement shall include the following:

   1. 1. General nature of violations,
   2. 2. Citation to statutes and rules alleged to be violated,
   3. 3. Disciplinary action to be taken,
   4. 4. Effective date of the disciplinary action if different from the date of the consent agreement,
   5. 5. Corrective action to be taken, and
   6. 6. Date to complete any corrective action.
2. B. A person entering into a consent agreement with the PMD shall waive the right to a formal hearing, rehearing, or judicial review of the matters contained in the consent agreement.
