1. A. When OLR takes adverse action against adoption agency applicant or adoption agency, OLR shall give the affected party written notice of such adverse action by first-class or registered mail.

2. B. For the purpose of this Section, the following are adverse actions:

   1. 1. Denial of an initial or renewal license, and
   2. 2. Suspension or revocation of a license.

3. C. The adverse action notice shall specify:

   1. 1. The action taken,
   2. 2. All reasons supporting such action,
   3. 3. The procedures by which the adoption agency may contest the action taken, and
   4. 4. Where the adoption agency may file an appeal.
