1. A. A license shall:

   1. 1. Be valid for the period of time specified on the license, and
   2. 2. Expire at midnight of the expiration date if the applicant does not apply for a renewal license in accordance with this Article.

2. B. To initiate the renewal of the license, the foster parent shall confirm:

   1. 1. With the licensing agency that he or she wishes to renew the license, and
   2. 2. The accuracy of or update the information via the Department's electronic database.

3. C. The foster parent shall cooperate with the licensing agency by:

   1. 1. Participating in and facilitating interviews necessary for the licensing agency to update the home study;
   2. 2. Assembling the documents needed to demonstrate ongoing compliance with licensing requirements;
   3. 3. Completing training, as described in R21-6-303 and R21-6-331 if applicable.

   4. 4. Cooperating with the completion of a Life Safety Inspection of the home as described under R21-6-304:

      1. a. By the licensing agency every year, and
      2. b. By OLR at least once every two years.
   5. 5. Providing a current health self-disclosure for each adult household member every year;
   6. 6. Obtaining a physician’s statement for the foster parent at least once every two years and providing a physician’s statement for other adult household members if determined to be necessary by OLR in accordance with R21-6-302;
   7. 7. Maintaining a current and valid fingerprint clearance card meeting Level One requirements for each adult household member; and
   8. 8. Signing the Statement of Understanding.
