1. A. The licensing agency shall notify OLR and the Child Placing Agency of any issues arising under R21-6-326.

2. B. Within 48 hours of the occurrence of an incident specified in R21-6-326, the licensing agency shall provide in writing to OLR and the Child Placing Agency:

   1. 1. A description of the incident, including the place, date, and time of occurrence;
   2. 2. The names and contact information for any persons involved in the incident;
   3. 3. The measures taken by the foster parent to address, correct, or resolve the incident; and
   4. 4. The action taken by the licensing agency in response to the incident, if applicable.
