1. 1. Primary care from a contracted PCP located within 15 miles or 45 minutes of the enrollee’s home;
2. 2. High profile specialty care services from a contracted SPC within 20 miles or 60 minutes of the enrollee’s home; and
3. 3. Inpatient care in a contracted hospital, or a contracted special hospital within 30 miles or 90 minutes of the enrollee’s home.

Each HCSO shall provide each enrollee member living in a suburban area within the HCSO’s service area the following:
