1. A. A claimant or their authorized representative shall advise in writing to the Commission and carrier or self-insured employer of the claimant’s current mailing address and place of residence. If a claimant files a workers’ compensation claim against an uninsured employer, the claimant shall advise the special fund division of the claimant’s current mailing address and place of residence.
2. B. An uninsured employer or their authorized representative against whom a claimant files a workers’ compensation claim shall advise in writing the special fund division of the uninsured employer’s current mailing address and place or places of business.
3. C. Providing the address of a claimant’s or uninsured employer’s attorney or authorized representative is not sufficient to meet the requirements of this Section.
