1. A. A used oil collection center shall request a used oil collection center identification number on a form provided by the Director pursuant to A.R.S. § 49-802(C) that contains all of the following:

   1. 1. The company name;
   2. 2. The name of the owner of the company;
   3. 3. The mailing address and telephone number of the company;
   4. 4. The location of the collection center; and
   5. 5. A description of the type of used oil activity at the company.
2. B. Within 30 days of receiving the completed form, the Director shall issue the identification number to the used oil collection center.
