1. A. An applicant for a transportation network company permit issued by the Department under A.R.S. § 28-9552, shall apply to the Department by:

   1. 1. Completing and submitting online the application form provided by the Department at www.azdot.gov;
   2. 2. Providing the full name and contact information of the applicant’s agent for service of process in this state;
   3. 3. Certifying that the transportation network company meets the requirements of A.R.S. Title 28, Chapter 30, Article 3;
   4. 4. Filing a legible illustration of the applicant’s trade dress; and
   5. 5. Paying a $1,000 application fee as provided under A.R.S. § 28-9552(A).
2. B. Upon receipt and acceptance of all required documents, fees, and certifications, the Department shall issue to an applicant a transportation network company permit.

3. C. The application fee paid to the Department under subsection (A) is refundable in full if the transportation network company permit application is:

   1. 1. Denied by the Department, or
   2. 2. Withdrawn by the applicant before the Department issues a transportation network company permit.
4. D. A transportation network company permit issued by the Department under this Section expires three years after issuance and may be renewed as provided under R17-5-903.
