1. 1. The following information about the certificate holder:

   1. a. Legal name as shown on the application submitted under R13-9-402;
   2. b. Birth date;
   3. c. Physical description including race, gender, height and weight, and eye and hair colors; and
   4. d. Name of the law enforcement agency from which retired;
2. 2. The statement, “Retired Law Enforcement Officer,” following the certificate holder’s name;
3. 3. A certificate number;
4. 4. The date of qualification;
5. 5. The title “Retired Law Enforcement Officer’s Certificate of Firearms Proficiency”; and
6. 6. A brief statement on the reverse side identifying the certificate and its purpose.

The Department shall issue a certificate of firearms proficiency to an individual who is eligible under R13-9-401 and submits the information and documents required under R13-9-402. The Department shall ensure that the certificate of firearms proficiency contains:
