If the family wishes to appeal a decision regarding their subsidized guardianship agreement, the Division of Children and Family Services Subsidized Guardianship Coordinator or designee will:

1. (1)

   1. (A) Direct the family to 25 CAR pt. 34 for information on administrative hearings if questions from the family are received.
   2. (B) Instructions regarding how to request an administrative hearing are included on CFS-435-H: Notice of Modification or Termination to Subsidized Guardianship Agreement; and
2. (2) Keep the family and Adoption Manager or designee informed to any further proceedings related to the appeal request.
