- (a) A child who is receiving Medicaid as a result of an adoption subsidy may continue to receive the subsidy if the child moves to or from another state.
(b) The Adoption Support Specialist will:
- (1) Contact the ICAMA Coordinator or refer an adoptive parent to the ICAMA Coordinator in the Division of Children and Family Services Adoption Services Unit; and
- (2) Provide the ICAMA Coordinator with the adoptive family’s new address, phone number, and the effective date of the move.
(c) The ICAMA Coordinator will:
(1)
- (A) Complete the ICAMA forms in relation to a child who is moving from the state and forward the forms to the new state of residence.
- (B) Forward the following:
(i) Form 6.01 (Notice of Medicaid Eligibility/Case Activation);
(ii) Form 6.02 (Notice of Action); and
- (iii) Form 6.03 (Change in Child/Family Status), if applicable; and
(2) Forward completed ICAMA forms to the Eligibility Unit in relation to a child who moves into the state:
- (A) Form 6.01 or a COBRA letter; and
- (B) Copy of the Adoption Subsidy Agreement.
