(a) The Family Service Worker will:
- (1) Notify the FSW supervisor immediately of the emergency placement change;
(2)
- (A) Notify the child’s birth parent or parents, Office of Chief Counsel attorney, and attorney ad litem by phone or in person within twenty-four (24) hours of the emergency placement change.
- (B) If the whereabouts of the parent are unknown, reasonable diligence to locate the parents must be made and documented;
- (3) Provide the attorney ad litem with the name, address, and telephone number of the new foster home or placement provider within twenty-four (24) hours of the emergency placement change;
- (4) Provide written notice to the child’s birth parent or parents within seventy-two (72) hours of the emergency placement change via the completed CFS-300: Parent Notification of Change in Placement;
- (5) Provide written notice within seventy-two (72) hours of the emergency placement change to the attorney ad litem via the completed CFS-300-A: Attorney Ad Litem/CASA, and Child Notification of Change in Placement;
- (6) Place a copy of the completed CFS-300-A: Attorney Ad Litem/CASA Notification of Change in Placement in the child’s record;
(7)
- (A) Update child placement information in CHRIS.
- (B) Updating the placement information will open a response window to notify the Eligibility Unit of the placement change; and
- (8) Document submission of the CFS-300: Parent Notification of Change in Placement and CFS-300-A: Attorney Ad Litem/CASA Notification of Change in Placement in the CHRIS document tracking screen.
- (b) The Family Service Worker Supervisor will conference with the FSW as necessary regarding change in placement.
