(a) The Family Service Worker will:
- (1) Conference with supervisor as necessary regarding change in placement;
- (2) Provide written notice to the child’s birth parent or parents at least two (2) weeks prior to any placement changes via the completed CFS-300: Parent Notification of Change in Placement;
- (3) Provide written notice to the child (if age appropriate), the foster parents, the child’s attorney ad litem, the child’s Office of Chief Counsel attorney, and the court at least two (2) weeks prior to any placement changes via the completed CFS-300-A: Attorney Ad Litem/CASA, and Child Notification of Change in Placement;
- (4) Place copies of the completed CFS-300: Parent Notification of Change in Placement and CFS-300-A: Attorney Ad Litem/CASA Notification of Change in Placement in the child’s record;

- (A) Select the out-of-home placement that best fits the needs of the child.
- (B)
(i) A child who has been identified as an exempted-from-finding underaged juvenile offender or sexual offender must not be placed in a foster home with other children, unless the child’s therapist provides written notification that the child is no longer a danger to other children.
(ii) Place any written notifications from the child’s therapist in the child’s hard copy file.
(C)
- (i) If the recommended placement is a facility, the facility must receive information regarding the allegations.
- (ii) Document in the recommended placement screen in CHRIS that facility received information regarding allegations for a child with an exempted-from-finding underaged juvenile offender or sexual offender;
- (6) Arrange for a preplacement visit;
- (7) Provide new address to attorney ad litem;
- (8) Inform age-appropriate child of the right to appeal a change in placement;
- (9) Update child placement information in CHRIS (updating the placement information will open a response window to notify the Eligibility Unit of the placement change); and
- (10) Document submission of the CFS-300: Parent Notification of Change in Placement and CFS-300-A: Attorney Ad Litem/CASA Notification of Change in Placement in the CHRIS document tracking screen.
- (b) The Family Service Worker Supervisor will conference with FSW as necessary regarding change in placement.
