The Family Service Worker or Health Services Specialist will:
(1)
- (A) Refer all children receiving out-of-home placement services for the Early and Periodic Screening, Diagnosis, and Treatment Program (EPSDT) via the EMS-694 “EPSDT” form to the child’s primary care physician (PCP), or other EPSDT providers after the initial screening and the comprehensive health assessment.
- (B) If it is the first time the child has seen a healthcare provider while in the out-of-home placement, a primary care provider must be selected;
- (2) Make an appointment with the PCP or other EPSDT providers for the EPSDT screening;
- (3) File a copy of the examination report upon receipt from provider;
(4)
- (A) Schedule all subsequent screenings according to established periodicity schedules.
- (B) The most recent periodicity schedules can be found in the Arkansas Medicaid provider manuals;
- (5) Ensure all essential treatment is provided and documented in CHRIS in a timely manner; and
- (6) Ask the medical provider to complete the CFS-352: Medical, Dental, Vision, Hearing, and Psychological Episodic at each subsequent examination.
