(a)
- (1) A comprehensive health assessment of each child shall be completed.
(2) The assessment shall be conducted under the supervision of:
- (A) A physician; and
- (B) A qualified mental health practitioner.
- (3) Whenever possible, the Family Service Worker, Health Services Specialist, family, and foster parents shall be involved in the assessment to ensure as much information about the child as possible is available to the examining professionals.
(b)
- (1) A written plan specifying any conditions requiring treatment, the recommended treatment, the schedule for treatment, the names of healthcare providers responsible for treatment, and the results of treatment as it occurs shall be completed and updated periodically.
- (2) This plan shall be provided to the child’s birth parents and foster parents.
(3) The birth parents shall be:
- (A) Informed about any medical treatment scheduled for their child; and
- (B) Involved in and informed about their child’s health care.
