The Family Service Worker will:
(1)
- (A) Go to “Workload”, “Case” in CHRIS and complete the automated “Family Strengths and Needs Assessment” found within the “Case Plan”, “Assessment”, section of CHRIS on the “Family” and “Child” screens within thirty (30) days from the “Open Date” established by the case connection.
- (B) This report will be printed from CHRIS and may be the result of several family sessions;
(2) Consider resources for the family within:
- (A) The Division of Children and Family Services;
- (B) Other Department of Human Services divisions;
- (C) State agencies; and
- (D) In the community;
- (3) Make a referral to another division or community resource, if appropriate;
(4) Open a case in accordance to:
- (A) 9 CAR § 40-402, protective/supportive services case opening; and
- (B) 9 CAR § 40-403 out-of-home placement services case opening;
- (5) Hold staffings, as needed, in accordance with 9 CAR § 40-506, case staffings;
- (6) Arrange for other services as needed;
- (7) Deliver services as identified in the CFS-6010: Case Plan;
- (8) Address the educational needs, including any special needs identified during the strengths and needs assessment, of the children in the home, and attach copies of report cards, etc., as required in the CFS-6010: Case Plan;
- (9) Conduct staffing to discuss closure; and
- (10) Close noncourt involved case when both the Family Service Worker and the family agree that services are no longer needed or that the needs of the family will best be met by one (1) or more referrals to other service providers.
