The Family Service Worker will:
(1)
- (A) Gather data from the family about the needs of the family and place the information in the Family Advocacy and Support Tool (FAST).
- (B) This information is located in the “Case Plan/Needs Assessment” section on the “Family Screen”;
- (2) Enter all necessary data in CHRIS to create a client screen for each household member;
- (3) Review all information in the contact automatically populated from “case connect” screen into the contacts screen for the newly opened case;
- (4) Ensure that any current protection plan with a nearing thirty-day reassessment date is noted and that a reassessment of the safety of the child occurs within thirty (30) days of the implementation of the current plan; and
- (5) File an affidavit for dependency-neglect for any plan which has been reassessed and it is determined that a substantial risk of harm to the health and safety of the child remains.
