(a) The agency shall keep a confidential case record for each child that includes the following:

1. (1) Demographic information;
2. (2) Plan of safe care, if applicable;
3. (3) A complete intake;
4. (4) Assessments;
5. (5) Consents, including consent for medical care and authority to place the child;
6. (6) Interstate Compact on the Placement of Children, Arkansas Code § 9-29-201 et seq., information, if applicable;
7. (7) Treatment plans and treatment plan reviews;
8. (8) Copies of legal documents, for example, birth certificate, Social Security card, court orders, or documentation of its attempts to obtain the documents;
9. (9) Physical exams and immunization records or documentation of its attempts to obtain the documents;
10. (10) Psychological reports, if applicable;
11. (11) Educational reports, if applicable;
12. (12) Disciplinary and incident reports, if applicable;
13. (13) Daily behavioral observations;
14. (14) Nightly visual observations;
15. (15) Medication and physician’s orders;
16. (16) Therapy progress notes;
17. (17) Physician notes;
18. (18) Documentation of casework services and child contact; and
19. (19) Discharge statement.
