Any agency licensed in Arkansas shall provide the following information to the Child Welfare Agency Licensing Unit prior to opening a branch office in Arkansas:

1. (1) The address, telephone numbers if available, and office hours for the branch office;
2. (2) The name or names, qualifications, and contact information of the person or persons responsible for the day-to-day operations and the child placement activities of the branch; and
3. (3) The name or names and contact information of the person or persons responsible for providing services in case of emergencies or child placement crises.
