- (a) The agency shall provide information to the adoptive applicants regarding the potential risks associated with adoption.
- (b) The agency shall provide a written statement of exclusion (for example, single parents or unwed couples) to adoptive applicants before a home study is conducted, if applicable.
(c) The following information, if available, shall be provided to adoptive parents regarding the child being considered for adoption:
- (1) Specific and accurate information about the needs and characteristics of the child;
- (2) The health and medical history of the child and the child’s biological family;
- (3) The health status of the child at the time of placement; and
(4) Genetic and social history of biological relatives, including:
- (A) Medical history;
- (B) Health status, if alive;
- (C) Cause and age of death, if deceased;
- (D) Height, weight, eye color, and hair color;
- (E) Levels of education and professional achievement;
- (F) Ethnic origins; and
- (G) Religion.
(d)
- (1) The agency caseworker shall ensure that at least two (2) face-to-face post-placement visits are made within six (6) months after the placement of the child.
- (2) One (1) of the visits shall be in the home of the adoptive family.
- (3) If a finalized decree of adoption has been issued, then post-placement visits are not required.
- (e) The agency shall have a plan for caring for children if their placement is disrupted before the issuance of a decree of adoption.
- (f) The agency shall offer supportive services to the adoptive family for at least six (6) months following placement.
