(c) The applicant for the proposed activity must provide the following information, at minimum, on a registration form prescribed by the district:

1. (1) The applicant's:

   1. (A) Legal name;
   2. (B) Mailing address;
   3. (C) Email address; and
   4. (D) Telephone number;
2. (2) The street address as denoted by the local 911 service, legal address, and driving directions of the proposed facility;

3. (3) If the owner of the proposed facility is other than the applicant, include a copy of the:

   1. (A) Rental or purchase agreement for the facility which contains the property owner’s:

(i) Legal name;

1. (ii) Mailing address;

   1. (iii) Email address; and

      1. (iv) Telephone number; and
   2. (B) Legal instrument giving the applicant the right to use the realty for a recycling activity;
2. (4) The service areas which the proposed facility will serve;
3. (5) A list of all recyclable or recovered materials to be diverted from the waste stream;
4. (6) The size of the proposed or existing facility and capacity proposed; and
5. (7) A written operating plan which establishes methods for compliance with all applicable sections of Arkansas Pollution Control and Ecology Commission, Solid Waste Management Rules, 8 CAR pt. 60.
