(b) The notice of intent shall include the following information:

1. (1) The name of the applicant;
2. (2) The applicant's address and telephone number;
3. (3) Whether the applicant is seeking a new or modified solid waste facility permit and the classification of the permit sought;
4. (4) The site of the proposed facility;
5. (5) A description of the geopolitical area to be served by the proposed facility, including population estimates by jurisdiction; and
6. (6) For landfills, confirmation from the Division of Environmental Quality that the applicant has requested a statement concerning the current and proposed solid waste landfill disposal capacity respective to the area and landfill class being proposed.
