All documents, reports, or correspondence created pursuant to activities addressed by this part, whether submitted to the Division of Environmental Quality or to a client, shall include:

1. (1) The following certification statement: I certify under penalty of law that this document and all attachments were prepared under my direction or supervision according to a system designed to assure that qualified personnel properly gather and evaluate the information submitted. The information submitted is, to the best of my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting false information. ; and
2. (2) The signature of the certified Phase I consultant or certified clandestine laboratory remediation contractor.
