(a)
(1) A tire generator shall provide written notice to the Division of Environmental Quality within thirty (30) calendar days of any material change that affects its:
- (A) Permit;
- (B) License; or
- (C) Registration.
(2) A material change includes any one (1) or more of the following:
- (A) The relocation of the place of business;
- (B) New locations where the business is conducted;
- (C) The closure of a business or business location;
- (D) A change in the:
(i) Name;
(ii) Mailing address;
(iii) Telephone number; or
- (iv) Email address; or
(E) Purchasing new equipment intended to be used for activities that are subject to the:
- (i) Permit;
- (ii) License; or
- (iii) Registration.
- (b) If a tire generator is registered with the Department of Finance and Administration to collect and remit rim removal fees, the tire generator must also provide written notice to the department within thirty (30) calendar days of any material change under this section.
