(a)

1. (1) A tire generator shall provide written notice to the Division of Environmental Quality within thirty (30) calendar days of any material change that affects its:

   1. (A) Permit;
   2. (B) License; or
   3. (C) Registration.

2. (2) A material change includes any one (1) or more of the following:

   1. (A) The relocation of the place of business;
   2. (B) New locations where the business is conducted;
   3. (C) The closure of a business or business location;
   4. (D) A change in the:

(i) Name;

1. (ii) Mailing address;

   1. (iii) Telephone number; or

      1. (iv) Email address; or

   2. (E) Purchasing new equipment intended to be used for activities that are subject to the:

      1. (i) Permit;
      2. (ii) License; or
      3. (iii) Registration.
