(a) An owner or operator of a tire processing facility shall submit to the Division of Environmental Quality an annual report by June 30 that summarizes information on the activities of the tire processing facility.
(b) The report shall be submitted to the division using Form TAP-5 (Used Tire Collection and Processing Facility Annual Report).
