(b) The beneficial use proposal shall include the following information unless otherwise specified by the division in writing:

1. (1) Legal description, address, and directions to the proposed beneficial use site;
2. (2) A map or aerial photograph indicating land use within a one-fourth (1/4) mile radius of the proposed beneficial use site which shall be of adequate scale to show all residences, structures, surface waters, public and private water supply sources, access roads, historic sites, and other existing human-made features relating to the beneficial use site;
3. (3) Maps indicating the existence of any regulated wetlands or floodplains on or adjacent to the beneficial use site;

4. (4) A site plan map that delineates:

   1. (A) Survey boundary locations;
   2. (B) The location of existing or proposed access roads;
   3. (C) Existing buildings and improvements including any staging and storage areas for incoming used tires;
   4. (D) General site topography; and
   5. (E) Existing and proposed drainage characteristics including any run-on/run-off control systems;

5. (5) A list of the tire generators including:

   1. (A) Name;
   2. (B) Location; and
   3. (C) Mailing address;
6. (6) Concurrence of the landowner indicating acceptance and responsibility of the used tires or residuals and an agreement to indemnify the division from any future liability resulting from the beneficial use project;
7. (7) Statement or statements that the beneficial use project complies with all applicable rules from any local regulatory body having jurisdiction over the beneficial use project to include any one (1) or more of the following: planning, building, code enforcement, or drainage departments;
8. (8) Any applicable environmental permits;

9. (9)

   1. (A) Project specifications and details including:

(i) A description of the proposed application;

1. (ii) The quantity of used tires anticipated to be used in the project;

   1. (iii) Construction methods and materials;

      1. (iv) Construction drawings and details; and
      2. (v) A project schedule.
   2. (B) An explanation shall be provided demonstrating how the engineering properties of these materials will be beneficial to the project; and
2. (10) An explanation of how the beneficial use project will comply with all applicable provisions of this part.
