A procedurally sufficient complaint must:

1. (1) Be in writing;
2. (2) Be signed;
3. (3) Include an affirmation that facts articulated in the complaint are true and correct to the best of the complainant’s knowledge under penalty of perjury;

4. (4) Clearly describe the alleged:

   1. (A) Incident;
   2. (B) Election irregularities; or
   3. (C) Illegalities;
5. (5) State approximately when the alleged violation or incident occurred;
6. (6) State the location or locations where the alleged activities occurred; and
7. (7) Provide any supporting facts surrounding the allegations.
