(a) The process to obtain a declaratory order is begun by filing a petition that provides:

1. (1) The name, address, telephone number, and signature of the person requesting the order; and
2. (2) A complete statement of the facts and circumstances applicable to that person, including the statutes, rules, or orders for which clarification is sought.
