(a) A verification of voter registration card shall contain the following information for the voter applying for the card:

1. (1) Applicant’s full legal name; and
2. (2) Registrant ID number from the electronic voter registration system maintained by the county clerk.

(b) A verification of voter registration card shall also contain the following:

1. (1) The name of the county issuing the card;
2. (2) The date of issuance;
3. (3) Color photograph of the voter applying for the card; and
4. (4) A statement establishing that the card is valid only for voting purposes.

(c) If permitted by the software provided by the Secretary of the State, the verification of voter registration card shall contain the following:

1. (1) A silhouette or outline of the county issuing the card; and
2. (2) A copy of the signature of the county clerk.
