(b)

1. (1) The office of Secretary of State shall:

   1. (A) Post a copy of the application on its website; and
   2. (B) Make a copy of the application available upon request.
2. (2) The Secretary of State shall also provide a copy of the application form to each of the seventy-five (75) county clerks in the State of Arkansas.

(c) The application for a verification of voter registration card may request the following information from an applicant:

1. (1) Full legal name;
2. (2) Residential street address where the applicant is registered to vote;
3. (3) Date of birth;
4. (4) Gender;
5. (5) Height; and
6. (6) Eye color.

(d)

1. (1) The application shall include an affidavit that he or she does not possess a card or document that would qualify as verification of voter registration as defined in the Arkansas Constitution, Amendment 51, § 13.
2. (2) The affidavit shall specifically include the definition of verification of voter registration.
