- (a) Applications for the State Crime Laboratory Student Loan Forgiveness Program will be submitted within thirty (30) days following the completion of two (2) consecutive years of employment.
(b) Completed applications will include all of the following documents:
- (1) A completed application form on the form approved by the State Crime Laboratory;
(2)
- (A) A statement on the student loan creditor’s letterhead that includes the total outstanding balance of the student loan.
- (B) The statement will include:
- (i) The student loan creditor’s:
- (a) (a) Name;
- (b) (b) Contact information; and
(c) (c) Address where payments should be sent; and
- (ii) Any special instructions for remitting payment on behalf of the pathologist;
- (3) The pathologist’s executed employment contract;
(4)
- (A) A letter from the State Medical Examiner stating that the pathologist has successfully completed a one-year probationary period.
(B) The letter from the State Medical Examiner is not required for applicants that have:
- (i) Been employed more than two (2) years; and
- (ii) Already received one (1) payment under the program; and
(5) Any other documents required by the Director of the State Crime Laboratory.
(c) Completed application packets will be forwarded to the director, or their designee, for review and approval.
- (d) The director’s decision to reward or deny an application is final.
(e)
- (1) The director will provide notice to the applicant when an application is approved or denied.
- (2) When an application is denied, the director will provide the applicant with a reason for denial.
(3)
- (A) An applicant whose application has been denied for failure to submit a completed application packet will be given ten (10) days from the date of denial to submit the required documents.
- (B) Documents submitted after ten (10) days may be denied at the discretion of the director.
