- (a) Enrollment agreements or contracts shall not be written for more than one (1) program of study.
- (b) When items are marked through so that changes can be made, each change shall be initialed by all parties.
(c) Schools shall provide and maintain enrollment agreements and/or contracts that contain not less than the following for three (3) years from the student’s last date of attendance:
- (1) The name of the school as licensed;
(2)
- (A) The location address of the school where the training is to be offered.
- (B) In the case of a traveling school, the certificate shall indicate the location of the school and that this is a traveling school license;
- (3) The name and address of the student;
- (4) The program title, tuition, and registration fee as approved by the State Board of Private Career Education;
- (5) The program length in clock or credit hours;
- (6) The student’s signature and date the document was signed;
- (7) The school official’s signature, title, and date the document was signed;
- (8) The start date and projected end date; and
- (9) If the student is a minor, the parent or guardian’s signature and date the document was signed.
- (d) Blanks are not allowed on enrollment agreements, complete with N/A, etc.
