(b) The application shall set forth and be accompanied by the appropriate documentation and fees:

1. (1) The name and location of the school;
2. (2) Information on the type of ownership;
3. (3) Parent company information;
4. (4) Completed forms for all partners or shareholders with ten percent (10%) or more interest in the school;
5. (5) Copy of enrollment application and/or student-school contract forms;
6. (6) Details on each proposed program to include, but not be limited to, such information as occupational demand, course titles, length, cost, and equipment available;
7. (7) A copy of a fire inspection report that has been completed within the past twelve (12) months;

   1. (A) One (1) copy of the current school catalog/bulletin, in printed form.
   2. (B) Determination or interpretation of school policy shall be based on the most recent catalog/bulletin on file with the State Board of Private Career Education;
9. (9) If applicable a sample copy of the completion certificate that will be awarded to students; and
10. (10) A copy of a student transcript.
