A licensee whose license is suspended shall apply for reinstatement of the license by providing the following:

1. (1) Approval from the Professional Licensure Standards Board of the reinstatement from suspension based on its determination that the applicant has:

   1. (A) Completed all conditions of the suspension; and
   2. (B) Paid the assessed fine; and
2. (2) For reinstatement from suspension requested before the year of renewal for the license, the applicant shall submit an application for reinstatement.
