(b)

1. (1) For each student diagnosed with diabetes who attends the school, the school district shall maintain a:

   1. (A) Copy of the student’s individualized healthcare plan;
   2. (B) List of school personnel who have volunteered and been trained to administer:

(i) Insulin;

1. (ii) Glucagon; or

   1. (iii) Both; and

   2. (C) Copy of the written authorization of the student’s:

      1. (i) Parent;
      2. (ii) Guardian; or
      3. (iii) Person acting in loco parentis.

2. (2) The list of volunteer school personnel and a copy of the written authorization shall be:

   1. (A) Updated annually; and

   2. (B) Attached to the student’s individualized healthcare plan.

3. (1) The list of volunteer school personnel shall include only the names of personnel who successfully completed the required training as set forth in 6 CAR § 154-203.

4. (2) The list of volunteer school personnel should be:

   1. (A) Published; and

   2. (B) Made known to all school personnel.

      1. (d) The principal of each school, in conjunction with each school nurse, shall properly maintain all such records.
