(a) The Department of Education will disseminate to districts, through the educational cooperatives, the student health record folders and health history forms.
(b) Parents shall complete a health history form prior to the physical examination/assessment.
(c) The school will file each student's health history form in the student health record folder.
(d) The school shall periodically update the student's record as additional medical data becomes available.
(e) The school shall maintain records showing the:
(1) Number of students screened;
(2) Number referred for treatment or further evaluation;
