- (v) Other opportunities to enrich the student’s success;
(3) Address academic deficits and interventions which may include without limitation:
- (A) Courses based on identified areas of academic deficit;
- (B) Point-in-time remediation, credit recovery, and tutoring;
- (C) Additional learning supports and transitional coursework; and
- (D) Other opportunities to support the student’s success; and
(4) Include college and career planning components based on a review of information appropriate for the student, including without limitation:
- (A) College and career readiness assessment data;
- (B) Interest inventories;
- (C) College and career planning tools;
- (D) Industry-recognized credentials or technical certifications; and
(E) Other postsecondary preparations.
- (c)
- (1) Each student's student success plan, or the student's individualized education program, if applicable, shall include the recommended sequence of courses for successful completion of a diploma pathway selected by the student.
- (2) A student success plan shall be sufficiently flexible to allow the student to, with the written approval of a parent, change his or her diploma pathway.
(3) A change made to a student's diploma pathway under this subsection shall be structured to ensure that the student will:
- (A) Meet the high school graduation requirements for the student's chosen diploma pathway or the requirements of the student's individualized education program, if applicable; and
- (B) Qualify for admission to a postsecondary educational institution or to enter the workforce.
- (4) Each student's student success plan shall be reviewed annually and revised as necessary to identify the courses to be taken each year until all required core courses are completed.
(5) Upon completion of the review required under subdivision (c)(4) of this section, the student's student success plan shall be signed by the:
- (A) Student;
- (B) Student’s parent, legal guardian, or person standing in loco parentis to the student; and
- (C) School counselor.
- (6) Before revising a student success plan under subdivision (c)(4) of this section, a school counselor or person acting in the equivalent role of a school counselor shall meet with the student's parent either in person or virtually to explain the possible impacts the revisions to the plan might have on the student's graduation requirements and postsecondary education goals.
- (7) Any revisions to a student success plan shall be approved in writing by the student's parent.
(8) A student success plan template and guidance on using available platforms and tools to meet the requirements of this section for use by public school districts and open-enrollment public charter schools is available on the Division of Elementary and Secondary Education’s website.
- (d) An IEP for a student with a disability, identified under the Individuals with Disabilities Education Act, 20 U.S.C. § 1400 et seq., meets the requirements of this section if the IEP:
- (1) Addresses academic deficits and interventions for students not meeting standards-based academic goals at an expected rate or level; and
- (2) Includes a transition plan that addresses college and career planning components.
