(b) Such application shall be in writing, verified under oath, and shall contain or be accompanied by the following:

1. (1) The name (and trade name, if any) and address or location of the principal office or place of business of applicant;
2. (2) A copy of applicant's authority granted by the United States Surface Transportation Board;
3. (3) A complete description of the type of service rendered by the applicant;

4. (4)

   1. (A) If applicant is a corporation, the state in which incorporated, and the name and address of its president and secretary.
   2. (B) If applicant is a partnership, the name and address of its partners;
5. (5) If applicant is not a resident of this state, a designation in writing of the name and address of a person maintaining a residence within this state upon whom service of process may be had in all actions arising in this state; and
6. (6) Remittance of the statutory filing fee.
