- (a) Regardless of the permit type, utility owners are required to secure a permit prior to the commencement of work.
- (b) A written permit application, including plans, shall be submitted to the appropriate District Engineer.
(c) The application and plans shall include, at a minimum, the following:
(1)
- (A) Two (2) sets of plans with crossing profiles, when applicable.
- (B) Plans printed on eleven inches by seventeen inches (11” x 17”) sheets are preferable.
- (C) Plans may be submitted in electronic form utilizing portable document format, if acceptable to the District Engineer;
- (2) Type and description of the facility;
- (3) Location with reference to centerline stations from Arkansas Department of Transportation plans or other well-known permanent landmarks;
(4) The measured distance from:
- (A) The right-of-way line;
- (B) The centerline;
- (C) Ditch lines;
- (D) Toe of slopes; and
- (E) The edge of pavement of the highway; and
- (5) A traffic control plan.
- (d) The utility owner shall also comply with the provisions of 27 CAR § 91-504 or 27 CAR § 91-505.
- (e) All permits shall constitute a binding contract, therefore, proxy applications, verbal, or unsigned requests will not be valid.
