The third-party testing fee schedule is as follows:

1. (1) Third-party tester application fee, one hundred dollars ($100);
2. (2) Third-party tester renewal fee, one hundred dollars ($100);

3. (3) Third-party examiner application fee (includes training):

   1. (A) Forty-hour course (required for new examiners), five hundred dollars ($500); and
   2. (B) Eight-hour course (required for previously trained examiners), one hundred dollars ($100); and
4. (4) Third-party examiner renewal fee, fifty dollars ($50.00).
