An applicant shall verify to the Office of Driver Services that the following information contained within records of the office is correct upon the renewal of the applicant’s driver’s license or identification card:

1. (1) The applicant’s full legal name;
2. (2) The applicant’s date of birth;
3. (3) The applicant’s gender information; and
4. (4) The applicant’s address of residence.
