The form of the notice of termination of cancellation of a certified policy, which shall be referred to as an SR-26, shall include, but shall not be limited to the following items:

1. (1) Name and current address of insured;

2. (2) Description of applicable vehicle:

   1. (A) Model year;
   2. (B) Name of manufacturer;
   3. (C) Model; and
   4. (D) Body type;
3. (3) Effective date of termination or cancellation;
4. (4) Name of insurance company;
5. (5) Date of execution; and
6. (6) Signature of authorized representative.
