When a court reports nonpayment of a judgment to the Office of Driver Services, the judgment certification shall contain, but shall not be limited to, the following information:

1. (1) Date and location of accident;
2. (2) Date of judgment;
3. (3) Amount of judgment;
4. (4) Name of plaintiff and defendant;
5. (5) Amount of damage to personal property of plaintiff;
6. (6) Monetary amount of damage resulting from personal injury or death of plaintiff;
7. (7) Monetary amount for care and loss of services of plaintiff; and
8. (8) Statement that judgment has remained unsatisfied more than thirty (30) days.
