Any written agreement filed with the Office of Driver Services as an exception to the security requirement should include, but shall not be limited to the following items:

1. (1) Conditions of the agreement;
2. (2) Date and location of accident;
3. (3) Date agreement was executed;
4. (4) Signature of all parties included in the agreement; and
5. (5) Statement that such agreement is for all claims of the creditor.
