Notice of the licensee’s right to a hearing shall contain the following information:
- (1) Date of accident;
- (2) Location of accident;
- (3) Last day in which the hearing may be requested in which the request will act as a stay on any further action;
- (4) A statement of the Office of Driver Services’ legal authority; and
- (5) A statement as to what will be considered at the hearing.
