(a) All requests for information from the records of Office of Motor Vehicle shall:

1. (1) Be in writing; and

2. (2) Include the:

   1. (A) Name and address of the person requesting the information; and
   2. (B) Intended use of the information.

(b) Improper use of personal information:

1. (1) Constitutes a violation of the Driver’s Privacy Protection Act of 1994, 18 U.S.C. § 2721 et seq.; and
2. (2) May subject the violator to civil damages or criminal penalties.
