(a)
- (1) Cases may either be closed as Goals Met or Goals Not Met.
- (2) Before case closure the VR counselor must review the case and ensure all required documentation has been entered into the case file.
- (3) All consumers must receive a thirty-day closure warning letter to provide them an opportunity to address any concerns about case closure.
(b)
(1) Once a case is ready for closure the VR counselor must:
- (A) Review the case; and
- (B) Complete the closure draft.
- (2) All cases must have a Closure Narrative addressing the conditions for closure.
- (3) Once this is complete, their supervisor will review the case and enter the closure date or send the case back for corrections.
(b) Goals Met.
(1) When a case is closed as Goals Met, this means that:
- (A) All planned services have been provided; and
- (B) The IPE goal has been achieved.
(2) At the time of closure both the consumer and VR counselor should be satisfied that:
- (A) All available services have been provided; and
- (B) The consumer can now live independently.
(c) Goals Not Met.
- (1) A case may close as Goals Not Met for a variety of reasons that indicate the consumer was not able to achieve self-sufficiency and independence during their case.
(2) The reason for closure must be:
- (A) Documented in the case file; and
- (B) Included in the closure narrative.
(3) Reasons for unsuccessful closure are:
- (A) Voluntary withdrawal of participation in services;
- (B) The consumer is deceased;
- (C) Unable to locate consumer or consumer has moved;
- (D) Consumer institutionalized; and/or
- (E) Consumer fails to cooperate.
