1. (a)

   1. (1) All applicants are entitled to participate in the preliminary needs assessment.
   2. (2) Needs assessment includes the most applicable assessment tool (e.g., Comprehensive Functional Assessment (CFA), Orientation and Mobility Evaluation, etc.) and is used to establish goals for the client.
   3. (3) This assessment should determine the nature and scope of services needed by the consumer in order to develop the IPE.

2. (b)

   1. (1) If the client is requesting independent living services, a CFA must be administered.
   2. (2) If the client is requesting limited training in a specific area, then the appropriate assessment tool is to be completed (e.g., Orientation and Mobility Evaluation, Assistive Technology, etc.).
   3. (3) Completion of the CFA or other appropriate assessment must be documented in AWARE.
3. (c) Assessments are to be administered in a face-to-face meeting.

4. (d) The assessment will be entered:

   1. (1) At the start of the case;
   2. (2) At closure; and
   3. (3) Annually on the anniversary date.
5. (e) All exceptions must be documented.
