(a)
- (1) All applicants are entitled to participate in the preliminary needs assessment.
- (2) Needs assessment includes the most applicable assessment tool (e.g., Comprehensive Functional Assessment (CFA), Orientation and Mobility Evaluation, etc.) and is used to establish goals for the client.
- (3) This assessment should determine the nature and scope of services needed by the consumer in order to develop the IPE.
(b)
- (1) If the client is requesting independent living services, a CFA must be administered.
- (2) If the client is requesting limited training in a specific area, then the appropriate assessment tool is to be completed (e.g., Orientation and Mobility Evaluation, Assistive Technology, etc.).
- (3) Completion of the CFA or other appropriate assessment must be documented in AWARE.
- (c) Assessments are to be administered in a face-to-face meeting.
(d) The assessment will be entered:
- (1) At the start of the case;
- (2) At closure; and
- (3) Annually on the anniversary date.
- (e) All exceptions must be documented.
