(a)
- (1) Outreach activities are to inform and educate community organizations in order to obtain referrals for individuals who may be eligible for Division of State Services for the Blind program services.
- (2) Activities include staying in touch with key community organizations that can identify and refer potential clients.
- (3) Outreach follow-up activities can include personal contacts, phone communications, news releases, mail-outs, special events, public service announcements (PSAs), etc.
- (4) All Division of State Services for the Blind counselors are responsible for conducting outreach.
(b) Outreach means that a counselor will:
- (1) Establish and maintain a good working relationship with potential referral sources;
- (2) Maintain efficient and accurate referral sources;
- (3) Respond to requests and schedules in a timely manner;
- (4) Make presentations to individuals and groups; and
- (5) Keep appointments and visits.
(c) The counselor should develop working relationships with:
- (1) Physicians;
- (2) Public officials;
- (3) County and state employees;
- (4) Area agencies on aging;
- (5) Aging network personnel;
- (6) Senior center staff and programs;
- (7) Health-related organizations;
- (8) The Division of Workforce Services;
- (9) County health nurses;
- (10) Department of Human Services staff;
- (11) Other potential referral agencies and sources in the respective area of service; and
- (12) Service organizations, such as Lions Clubs.
