1. (a) If a member applying for retirement selects a straight life benefit, then the member shall be required to furnish the spouse’s acknowledgment of that selection on a form provided by the Arkansas Public Employees’ Retirement System.

2. (b)

   1. (1) An application shall not be denied because a spouse refuses to sign the form.
   2. (2) However, a proof of notice to the spouse of this benefit selection shall be placed in the member’s file.
